How to Schedule and Prepare for Your First Appointment

There are FOUR main steps involved in preparing for your first appointment.

INFORMATION: Complete the new patient forms: You'll see that these are probably the most comprehensive forms you've ever completed for a doctor's visit. This is because our approach will be highly comprehensive, and the more data we have, the better we can fine-tune the treatment plan and help you accomplish your health goals.
PREPARATION: Bring copies of your previous/recent laboratory tests for your file: If you have copies of your previous/recent lab tests, bring these so that we can review them and put them in your file. Bring a copy to leave with Dr Vasquez--we will not have time to go through the process of copying these during your visit. Bring a copy that you can leave. We will use your previous results to determine which new tests need to be performed or repeated. Bring your current medications and nutritional supplements: We will need to review your current intake of medications and supplements.
TIMING: Plan to arrive on time: Dr Vasquez is incredibly busy with writing books, doing research, and giving seminars to doctors. We must strive to begin on time, and we will make a strong effort to end on time. On the other hand, each visit will be as complete as possible, and we will not feel rushed, even though we have much to accomplish in a finite amount of time. Coordinate a specific time for the appointment: Call (817) 870-0212 to schedule your appointment; leave a message if there is no answer and your call will be returned within 24 hours. DEPOSIT: All appointments are held with a $60 deposit payable on-line with credit card: Of course, this $60 is applied to your visit fee. The fee is entirely refundable if your appointment is cancelled with more than 24 hours notice. If you cancel/reschedule your appointment without giving at least 24 hours notice, the deposit is not refundable. Why? You've probably noticed that better restaurants and even some schools now require deposits for reservations. The deposit solidifies our agreement and commitment to work together at a specific time. Soon after you've made the deposit, you will receive a receipt and confirmation email or phone call to schedule your appointment.
